1. Request an Audiology Referral to a CCN Provider:

Contact your Primary Care Provider (PCP) or VA healthcare team

Call the Community Care Network (CCN) team at (210) 949-3850

Visit MyHealth.VA.gov and log in to your account to request a referral

2. Hear in Texas is Your Authorized CCN Provider:

3. Schedule Your Appointment With Hear in Texas:

Once you have your referral number (within two to three days), call Hear in Texas to schedule your appointment

Your VA Health team will enter this information into your chart to coordinate with your CCN provider

4. Your Authorization Letter:

After scheduling your appointment, you will receive an authorization letter. This letter is your approval for community care and includes:

Information about your approved in-network community provider

A description of the care you are authorized to receive
